What We’ll Need To Provide Your FREE Quote Or Review

If you currently have a benefits program, we will require the following information to provide a FREE comprehensive review of your current plan and/or to provide quotes:

  • Claims Report showing the most recent two years of PAID claims versus PAID premium by line of benefit
  • Current rates and renewal rates by line of benefit
  • Copy of your most current billing statement
  • Copy of an employee booklet for each class or division
  • Completed Employee Information Listing
  • List of any employees currently on LTD or WSIB including date of disability, diagnosis, and expected return date.

Most of this information is available in your most recent renewal document from your carrier.
